There is a SURVEY that all of us need to take, that deals with the services available in the state of Nebraska. You will want to be a part of this! Please pass it along to your families, professionals, or friends that you know! If you are involved daily with the life of a child birth to age 21, who has intellectual and/or developmental disabilities then this survey is for YOU! We need all surveys in by May 15!! If you know someone who doesn’t have a computer, I attached the hard copy at the bottom for your convenience! I just ask that you let me know how many you handed out or emailed out so we can try to keep a somewhat accurate count of who did the survey! Thanks.

YOU are NEEDED to take :
“A Survey of Services for Nebraska Families of Children Birth-21 with Intellectual &/or Developmental Disabilities”
The overall goal of this study is to identify family support options available in various parts of the state of Nebraska for NE families that have a child, birth to age 21, with intellectual& /or developmental disabilities. The results of this survey will be used to guide the Nebraska Planning Council on Developmental Disabilities in identifying gaps & barriers in what opportunities are available, the quality of services available, & to develop a plan that will result in systems changes that will improve both quality & availability of services.
The survey will not gather any identifying information, and is therefore completely anonymous!
This survey is being conducted as a joint project between The Arc of Nebraska and the Munroe-Meyer Institute at the University of Nebraska Medical Center.
